St. Luke's University Health Network is seeking an ACT/FACT Certified Peer Specialist to join a multidisciplinary team. The role focuses on outreach, monitoring, assessment, and service planning to support recovery and community integration for participants.

The position requires certification as a Certified Peer Specialist in Pennsylvania, a valid driver’s license, and willingness to work evenings and weekends as needed. Training and supervision are provided within a coordinated care setting.

Qualifications

  • Must hold and maintain Certified Peer Specialist certification through the Pennsylvania Certification Board.
  • WRAP (Wellness Recovery Action Plan) certification preferred.
  • Must possess and maintain a valid driver’s license along with a driving record in compliance with St. Luke's University Health Network policy.

Responsibilities

  • Meet with ACT participants—in their homes and other community venues—to provide in vivo services which improve the participants’ quality of life and maintain community tenure.
  • Provide medication service—including delivery of medications and observation of ingestion—as directed by the team psychiatrist, nursing staff, and Team Leader.
  • Provide service coordination for an assigned group of clients: develop, write, implement, evaluate, and revise treatment goals; offer individual and supportive counseling.

Education

High School Diploma or equivalent
Certified Peer Specialist certification
Dual CPS/CRS certification welcome
