Resources for Human Development (RHD) in Bethlehem, PA, seeks a compassionate Case Manager with CADC certification to join the Lehigh Valley Assertive Community Treatment (ACT) team. The role focuses on recovery-oriented, community-based services and collaboration with a multidisciplinary team.

You will help participants set recovery goals, connect them with resources, and advocate for access to needed services. Travel throughout the Lehigh Valley is involved, and daily pay options are available.

Qualifications

• Current Certified Alcohol and Drug Counselor (CADC) certification is required.
• Two years of experience supporting individuals in behavioral health, addiction recovery, homelessness services, or related programs is preferred.
• Intermediate computer skills and ability to complete electronic documentation.
• Strong interpersonal, communication, and customer service skills.
• Ability to work independently and as part of a multidisciplinary team.
• Valid driver's license and ability to travel throughout the community as required.

Responsibilities

• Work as part of a multidisciplinary ACT team providing holistic, recovery-focused services to participants.
• Assist individuals in developing and achieving goals outlined in their recovery plans.
• Connect participants to community resources, educational opportunities, healthcare services, housing supports, and recovery programs.
• Advocate for participants to ensure access to needed services and supports.
• Conduct substance use screenings and support participants in achieving recovery goals.
• Utilize CADC knowledge and skills to promote recovery, relapse prevention, and treatment engagement.
• Coordinate referrals to substance use treatment providers, recovery services, and community supports.
• Develop and maintain relationships with community agencies and service providers.
• Provide community-based services throughout the Lehigh Valley service area.
• Transport participants to appointments and community activities when needed.
• Complete assessments, service documentation, and reports in accordance with organizational and regulatory requirements.
• Identify service gaps and recommend strategies to enhance participant outcomes.
• Support participants in achieving greater independence, stability, and quality of life.
