Qualifications

  • Current CADC certification required.
  • Two years experience in behavioral health or related fields.
  • Intermediate computer skills and ability to complete electronic documentation.
  • Strong interpersonal and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver's license and ability to travel regionally.

Responsibilities

  • Work on a multidisciplinary ACT team providing holistic recovery services.
  • Assist participants in developing goals in their recovery plans.
  • Connect participants to community resources, housing, healthcare, education, and recovery programs.
  • Advocate for access to needed services and supports.
  • Conduct substance use screenings and support recovery goals.
  • Utilize CADC knowledge to promote recovery and treatment engagement.
  • Coordinate referrals to substance use treatment providers and community supports.
  • Develop and maintain relationships with community agencies and service providers.
  • Provide community-based services throughout the Lehigh Valley.
  • Transport participants to appointments and activities as needed.
  • Complete assessments and documentation per organizational and regulatory requirements.
  • Identify service gaps and recommend improvements.
  • Support participants in achieving independence and quality of life.

What You'll Do

As a Case Manager, you will:

  • Work as part of a multidisciplinary ACT team providing holistic, recovery-focused services to participants
  • Assist individuals in developing and achieving goals outlined in their recovery plans
  • Connect participants to community resources, educational opportunities, healthcare services, housing supports, and recovery programs
  • Advocate for participants to ensure access to needed services and supports
  • Conduct substance use screenings and support participants in achieving recovery goals
  • Utilize CADC knowledge and skills to promote recovery, relapse prevention, and treatment engagement
  • Coordinate referrals to substance use treatment providers, recovery services, and community supports
  • Develop and maintain relationships with community agencies and service providers
  • Provide community-based services throughout the Lehigh Valley service area
  • Transport participants to appointments and community activities when needed
  • Complete assessments, service documentation, and reports in accordance with organizational and regulatory requirements
  • Identify service gaps and recommend strategies to enhance participant outcomes
  • Support participants in achieving greater independence, stability, and quality of life

Qualifications
Required
  • Current Certified Alcohol and Drug Counselor (CADC) certification
  • Two years of experience supporting individuals in behavioral health, addiction recovery, homelessness services, or BHH/IDD programs
  • Intermediate computer skills and ability to complete electronic documentation
  • Strong interpersonal, communication, and customer service skills
  • Ability to work independently and as part of a multidisciplinary team
  • Valid driver's license and ability to travel throughout the community as required
Preferred
  • Bachelor's Degree in Human Services or a related field
  • One year of case management experience
  • Experience working with individuals experiencing serious mental illness and co-occurring substance use disorders
  • Knowledge of community resources and recovery-oriented services
  • ACT, intensive community-based services, or behavioral health experience
Schedule
  • Full-Time Position
  • Community-based work throughout the Lehigh Valley region
  • Monday through Friday schedule with flexibility based on participant and program needs
  • Some evening, weekend, and location flexibility may be required
Physical Requirements
  • Ability to lift up to 50 pounds occasionally
  • Ability to sit, stand, walk, bend, reach, kneel, crouch, and climb stairs as needed
  • Ability to travel regularly throughout the community
  • Ability to transport program participants safely as required
  • Ability to perform essential job functions with or without reasonable accommodation
