Job summary

Trilogy, Inc. in Chicago, IL is seeking a Recovery Counselor - Substance Use Specialist (ACT Magnolia). The role involves assessing client needs across home, community, and office settings, providing motivational, strengths-based counseling, and linking clients to community services for recovery from mental illness and co-occurring addiction.

A 75% focus on community services is expected. The position requires a degree or relevant experience, valid IL driver's license, and credentials

Qualifications

• Bachelor’s degree or HS diploma with 2 years of supervised clinical experience in mental health.
• Minimum one year of experience working with clients with substance abuse.
• Valid Illinois driver’s license with daily access to a vehicle and $100,000/$300,000 liability insurance
• Certification CADC/CRADC/CSADC/CAAP/CARS through IAODPCA or must obtain within 6 months of hire
• Ability to work evenings and weekends as directed.
• Experience in community-based services preferred.

Responsibilities

• Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits.
• Coordinate the establishment of an individual recovery plan with the client and support system.
• Facilitate education around substance use, dual diagnosis risks, for clients and team members.
• Provide one-on-one services focusing on substance use goals.
• Acquire a SAS credential within 12 months of hire.
• Link clients to community substance use treatments and higher levels of care as needed.
• Assist clients in identifying signs of de-compensation and develop coping skills.
• Help clients with independent living skills including hygiene, housekeeping, nutrition, shopping.
• Educate and assist clients with entitlements (SSI/SSDI/Medicare/Medicaid/LINK).
• Accompany clients to appointments and provide support for housing, budgeting, and medications.
• Advocate for clients and empower self-advocacy.
• Communicate effectively within the multi-disciplinary team and complete additional duties as assigned.
