Trilogy's Reentry Services in Chicago seeks a Recovery Counselor to assess the client’s situation and plan treatment through home, community, and office visits. The role delivers client-centered, strengths-based, and trauma-informed services to individuals with severe mental illnesses and co-occurring disorders, with a focus on reducing homelessness risk.

Qualifications

• Bachelor's degree or higher in a human-services field or related area with relevant supervised experience.
• Experience providing community-based mental health services and trauma-informed care.
• Ability to coordinate with multiple agencies and document care plans.

Responsibilities

• Assess client needs and plan treatment through home, community, and office visits.
• Provide client-centered, strengths-based services with trauma-informed approach.
• Develop individual recovery plans with clients, families, and care providers.
• Complete daily notes and maintain Trilogy documentation standards.
• Move clients toward appropriate care levels and connect to additional services.
