Qualifications

• At least one year experience in substance abuse treatment or related field.
• Strong documentation, organizational, and communication skills.
• Must have basic computer skills to enter data and use EHR systems.

Responsibilities

• Coordinate continuing recovery services for clients returning to the community after CSS program.
• Perform case management planning for housing, job searches, and medical/social services.
• Coordinate continuing care referrals by building relationships with community providers.
• Work with clinical staff to ensure smooth transition from residential to continuing care.
• Collect data and report program outcomes as directed.
