Job summary

Behavioral Health Network, Inc. seeks a Counselor/Case Manager to support individuals in Clinical Stabilization Services (CSS).

You will provide direct counseling, manage assigned caseloads, and develop comprehensive assessments and treatment plans to promote stabilization and recovery. You will coordinate discharge planning, connect clients with outpatient care, housing, and community resources, and document progress in the electronic health record with a trauma-informed, patient-centered

Qualifications

• Direct counseling, case management and recovery support in CSS.
• Manage caseload and build therapeutic relationships for stabilization.
• Complete admission/discharge docs and develop treatment plans, safety plans and relapse-prevention strategies.

Responsibilities

• Coordinate continuing-care and discharge planning with outpatient services.
• Facilitate psychoeducational and recovery-focused groups; document participation.
• Participate in multidisciplinary treatment-team meetings to ensure high-quality care.
• Maintain HIPAA and 42 CFR Part 2 compliance in all records.
