Job summary

Spectrum Health Systems in Westborough, MA is seeking a full-time entry-level position to coordinate continuing recovery services and provide aftercare counseling for clients transitioning from the CSS program.

The role focuses on housing, job searches, medical and social services planning, and collaboration with community providers to support clients’ ongoing recovery journey.

Qualifications

• High school diploma or equivalent required.
• At least one year experience in substance abuse treatment or related field.
• Strong documentation and communication skills.
• Basic computer skills to enter data and use electronic health records.

Responsibilities

• The coordination of continuing recovery services for clients returning to the community after completion of the CSS program and for providing aftercare counseling services to clients on campus.
• Performing case management activities based on clients' continuing care plans regarding initial housing, job searches, and establishment of medical services, social services, education, support services, and services from other community providers.
• Coordinating continuing care recovery referrals by developing positive relationships with community providers and including scheduling of initial appointments.
• Working closely with the Clinical staff in the CSS program to ensure the clients transition smoothly from the residential portion of treatment to the continuing care program.
• Collecting data and program success rates as directed.

• Minimum of a high school diploma or equivalent
