Job summary

HopeLink Behavioral Health seeks a Transition Specialist to lead psycho-educational groups in its Psychosocial Day Programs, coordinating care with county boards, healthcare providers, and educators to support recovery and independent living. The role emphasizes client empowerment and skills development.

Responsibilities include developing recovery-focused plans, teaching health and wellness curricula, and coordinating services to help clients transition to school or volunteer placements.

Qualifications

• Strong clinical written communication and presentation skills.
• Empathy toward clients served.
• Self-starter who can work independently or in a team.
• Experience with EHR or ability to learn EHR software.

Responsibilities

• Develop Transition Plans focused on recovery, health, and independence.
• Deliver psycho-educational curricula in group settings.
• Provide direct skill teaching in program and community settings.
• Offer individual/group counseling including dual diagnosis-focused services.
