Qualifications

• Bachelor's degree in Social Work, Psychology, or related field with experience serving adults with mental illness or substance use.
• QMHP-E required; QMHP preferred.
• Strong written communication and presentation skills; quick rapport with clients and providers.
• Self-starter who can work independently and as part of a team.
• Experience with EHRs or ability to learn quickly, Credible EHR a plus.

Responsibilities

• Develop person-centered Recovery/Transition Plans to empower clients.
• Deliver psycho-educational curricula to improve health and wellness.
• Provide direct skill teaching in various settings, including community.
• Offer individual/group counseling and dual-diagnosis focused services.
• Coordinate with colleges/universities to support school enrollment.
• Complete clinical documentation per policies and standards.
• Provide care coordination among providers to support recovery.
