Job summary

HopeLink Behavioral Health is seeking a Transition Specialist to run psycho-educational groups across its day programs, develop recovery-oriented transition plans, and coordinate care with partners to support clients’ well-being and independence.

The role emphasizes building client skills for school or volunteer placements, delivering individual and group counseling, and collaborating with educational and community providers to ensure successful outcomes.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related field.
• QMHP-E certification required; QMHP certification preferred.
• Strong clinical written communication and presentation skills.
• Ability to work independently and as part of a team.
• Experience with EHR systems (Credible preferred).

Responsibilities

• Develop Recovery/Transition Plans that are person-centered.
• Develop psycho-educational curricula to improve health and wellness.
• Provide direct skill teaching in various settings.
• Provide individual/group counseling and dual diagnosis-focused services.
• Assist clients in moving to volunteer placements and education.
• Provide crisis intervention as needed.
• Coordinate services with local colleges/universities for enrollment.
• Clinical documentation per policies and standards.
• Provide care coordination between providers to support recovery.
• Improve health and wellness for clients with complex needs.
• Screen for additional services and coordinate referrals.
• Demonstrate flexibility to work outside business hours.
• Provide opportunities that promote client leadership and advocacy.
• Adapt to varying duties including evenings and weekends.
