Job summary

The Dimock Center is seeking a Care Coordinator for the ASKIA program to support adult males in a co-occurring enhanced residential rehabilitation setting. You will build relationships, assess needs, and coordinate services toward recovery and housing stability.

Within the clinical team, you will develop care plans, link clients to housing, healthcare, employment resources, advocate on their behalf, and assist with discharge and aftercare while maintaining confidentiality.

Qualifications

• Bachelor’s degree in human services, psychology, sociology, health services, or a related field.
• At least one year of experience in a substance use treatment program.
• Experience with individuals facing substance use and/or mental health challenges or homelessness.
• Strong verbal and written communication skills.
• Strong interpersonal skills and ability to build rapport across diverse backgrounds.
• Ability to navigate community resources and coordinate multiple services.
• Strong organizational skills and ability to meet deadlines.
• Sound judgement in challenging or crisis situations.
• Teamwork and ability to work independently when needed.
• Proficiency with Microsoft Word, Excel, and PowerPoint.

Responsibilities

• Build and maintain trusting, professional relationships with clients and engage them in treatment.
• Meet with clients regularly to assess needs and support aftercare planning.
• Conduct comprehensive assessments to identify strengths, needs, barriers, and available supports.
• Develop individualized care plans with clients and the interdisciplinary treatment team.
• Coordinate referrals to medical, behavioral health, housing, employment, financial, legal, and community resources.
• Advocate for clients navigating health and human service systems.
• Identify social determinants of health affecting treatment and transition.
• Coordinate care with primary care and behavioral health providers.
• Assist with housing, benefits, financial assistance applications.
• Discharge and aftercare planning to ensure continuity of care.
• Complete clinical and administrative documentation accurately.
