Qualifications

• Valid Indiana driver’s license and clean driving record may be required by policy.

Responsibilities

• Assist clients to access recovery-oriented treatment and supports across systems.
• Accompany clients to appointments, meetings, and community services as needed.
• Document services provided in electronic health records accurately.
• Provide crisis management and de-escalation as part of care.
• Apply Recovery Model principles in daily interactions with diverse individuals.
