Job summary

Live Well Homecare Agency, LLC is seeking a Peer Recovery Coach to support individuals in sustained recovery. You will deliver one-on-one coaching and facilitate recovery-focused groups, using a person-centered, holistic approach.

You will connect participants to treatment providers and community resources, reinforce care plans, document services, and collaborate with health and social service partners to advocate for recovery-oriented care.
