Job summary

Promises Behavioral Health in Texas is seeking a dedicated Full-Time Case Manager with weekend availability to join our team. You will coordinate services, support residents, and facilitate transition to continuing care and sober housing after discharge.

You will conduct screenings, develop individualized service plans, and deliver psychoeducational and motivational sessions, while maintaining accurate records and collaborating with care teams.

Qualifications

• Strong clinical assessment and case management skills.
• Ability to document accurately in patient records.
• Knowledge of substance use and mental health services.
• Familiarity with DHS/JCAHO standards and regulatory requirements.

Responsibilities

• Screen patients through intake and assessment.
• Develop individualized service plans and coordinate referrals.
• Provide at least three case management sessions per patient weekly.
• Assist with recovery planning and transition to continuing care and sober housing.
