Qualifications

Position Overview:

The Case Manager provides comprehensive, person-centered services to individuals with substance use disorders, primarily participants of the STAR (Support Team for Addiction Recovery) Program. Based in our Newton office with community-based outreach, this role supports individuals in addressing barriers to recovery and improving overall quality of life through coordinated access to treatment, healthcare, housing, employment, education, and other support services. Working collaboratively with a Peer Recovery Specialist and other service providers, the Case Manager helps participants develop and achieve recovery goals while promoting self-sufficiency, wellness, and long-term recovery.

Qualifications

• Bachelor’s degree in health, psychology, social work, education, or other behavioral health professions, required.
• Minimum of three years of experience providing services or support to individuals with substance use disorders, behavioral health needs, or other high-need populations.
• Demonstrated knowledge of substance use disorders, recovery principles, and community-based treatment resources.
• Knowledge of formal and informal community systems, service networks, and referral resources.
• Ability to build rapport and effectively engage individuals from diverse backgrounds.
• Excellent interpersonal communication, documentation, organizational, and computer skills.
• Ability to work both independently and collaboratively as part of a multidisciplinary team.
• Must possess a valid NJ driver's license in good standing, maintain a safe driving record, carry adequate automobile insurance, and have reliable transportation.
• Ability to travel throughout the community to meet with clients and community partners.
• Flexibility to work occasional evenings or weekends as needed.
• Bilingual English/Spanish skills are a plus.

Position Details:
