Job summary

Northeast Recovery is seeking a full-time Case Manager for our Spring Hill Recovery outpatient facility in Quincy, MA. The role focuses on coordinating care for individuals with complex medical and behavioral health needs, developing treatment plans, and working with families and collateral contacts.

Schedule is Monday through Friday, with a goal of quality, compassionate case management. The ideal candidate will have 3–5 years of case management experience in behavioral health, and an LADC-2

Qualifications

• Bachelor's degree preferred; LADC-2 certification preferred.
• 3–5 years of case management experience in a behavioral health setting.
• Knowledge of federal, state and municipal regulations related to addiction treatment.

Responsibilities

• Conduct groups on psycho-educational and individual case management as directed.
• Complete intake assessments.
• Conduct treatment team meetings and family sessions.
• Manage and handle assigned caseload assessing clients' most urgent needs.
• Perform an in-depth mental or physical health analysis to develop a plan and resources.
• Consult with external agencies to provide support services and resources.
• Maintain comprehensive patient records of progress throughout treatment.
• Prepare a comprehensive discharge plan based on client needs.
• Other duties as required.
