Job summary

Regard Healthcare in Massachusetts is seeking a Case Manager for its Regard Recovery System of Care. Under supervision, the Case Manager advocates for clients, addresses immediate needs, coordinates services, and helps ensure safety and well-being.

The role requires collaboration with therapists and interdisciplinary teams, timely care planning, and strong communication. This on-site position demands compassion, independence, and solid organization to support adults in recovery.

Qualifications

• High school diploma or GED required; Bachelor's degree preferred.
• No felony or substance abuse convictions within the past five years.
• Must have 2 years sobriety if in recovery and active participation in a self-help program.
• At least 1 year of healthcare experience, preferably in mental health or substance use treatment.

Responsibilities

• Complete case management treatment plans within 48 hours of client admission.
• Coordinate care with primary therapists and interdisciplinary teams.
• Arrange transportation to approved medical appointments.
• Assist clients with employment-related documentation, including disability and FMLA.
• Communicate with probation officers, courts, and judicial staff to support discharge planning.
• Develop and coordinate aftercare plans with referrals to IOPs, psychiatrists, therapists.
• Participate in daily staff meetings and weekly discharge planning meetings.
• Report client concerns and program issues to the supervisor promptly.
• Complete and submit incident reports and required documentation timely.
• Adhere to ethical guidelines and state regulations.
• Demonstrate flexibility and adaptability in a dynamic work environment.
