Job summary

Lighthouse Mission Ministries in Washington, DC, seeks a Men’s Recovery Program Case Manager to support residents through the recovery process. You will manage a caseload, develop goals, monitor progress, and coordinate with staff and community partners to ensure access to needed resources.

The role requires flexibility for evenings, weekends, and holidays, with a compassionate, Christ-centered approach and a commitment to helping residents achieve lasting stability and self-sufficiency.

Qualifications

• Minimum of one year of experience in social services, particularly with individuals experiencing homelessness or in rescue mission settings.
• Active faith life and alignment with Lighthouse Mission's Statement of Faith and Personal Covenant.
• Knowledge of homelessness, poverty, addiction, and recovery issues.

Responsibilities

• Provide individualized case management to residents throughout participation in the Recovery Program.
• Maintain an assigned caseload and adapt to census changes.
• Develop, monitor, and update service plans with residents and the team.
• Conduct regular one-on-one meetings and document progress and outcomes.
• Complete timely case notes and assessments per standards and funding requirements.
• Provide crisis intervention and de-escalation support as needed.
• Coordinate with partners to connect residents with resources for long-term recovery.
• Liaise with outside agencies to promote referrals and continuity of care.
• Represent the program professionally in community meetings and with partners.
