Wise Path Recovery in Williamson, West Virginia, seeks a compassionate Case Manager to coordinate services, navigate community resources, and advocate for clients’ long-term recovery. You will work within a multidisciplinary team to support discharge planning and transitions of care.

The role emphasizes building trusting relationships, maintaining professional boundaries, and documenting progress in the EHR. CPR/First Aid certification or ability to obtain is required within 30 days of hire.

Qualifications

• High school diploma or equivalent required.
• Minimum 1 year of case management or related experience in behavioral health.
• Knowledge of community resources and ability to document in EHR.
• Strong communication, organization, and follow-through skills.

Responsibilities

• Provide comprehensive case management for clients with moderate to high clinical needs.
• Coordinate discharge planning and transitions of care to support continuity.
• Facilitate interdisciplinary case reviews and communicate progress, barriers, and recommendations.
• Assess client needs and identify barriers to treatment, discharge, or recovery.
• Connect clients with housing, employment, transportation, insurance, benefits, healthcare, and community resources.
• Build relationships with referral sources and community partners.
• Maintain compliant documentation in the EHR and participate in quality initiatives.
