Job summary

Gaudenzia is seeking a Case Manager for the RAP Program in Washington, DC. The role requires a bachelor’s degree in a clinical discipline and multiple years of case management experience across levels I–III.

Qualifications

• Bachelor's degree in a generally recognized clinical discipline is required.
• Bachelor's degree and relevant clinical knowledge preferred.
• Experience requirements vary by Case Manager level (I-III).
• Must maintain valid driver's license.
• Ability to coordinate with agencies and resources.

Responsibilities

• Treat all clients and staff with dignity and respect.
• Attend all team/staff meetings as required by Supervisor.
• Role model pro-social behavior.
• May conduct intake interviews as needed.
• Manage a caseload per program requirements and state regulations.
• Complete progress notes within 24 hours of service delivery.
• Coordinate with agency resources for thorough treatment and discharge plans.
• Maintain positive relationships with providers and referral sources.
• Assist clients in connecting to resources to reach goals.
• Arrange access to public assistance and medical services as needed.
• Arrange medical and psychiatric appointments and medications when appropriate.
• Refer clients to vocational and educational programs.
• Develop discharge plans addressing clients' significant needs.
• Maintain post-discharge contact for coordination with services.
• Ensure compliance with all guidelines and maintain licensure/certifications.
• Keep a valid driver’s license.
• Other duties as assigned by Supervisor.
