Job summary

Gaudenzia, Inc. is seeking a dedicated Case Manager to promote recovery through comprehensive case management and connection to community resources.

Qualifications

• Bachelor's degree in a human services discipline required.
• 2–3 years' case management and/or drug and alcohol counseling experience.
• Ability to manage a caseload and coordinate with community resources.

Responsibilities

• Treat all clients and staff with dignity and respect.
• Attend all team/staff meetings as required by Supervisor.
• Role model pro-social behavior.
• May conduct intake interviews and manage caseload per program requirements.
• Complete progress notes for all client-related activities within 24 hours of service delivery.
• Coordinate with agency and community resources to ensure thorough treatment and discharge plans.
• Establish and maintain positive relationships with service providers and referral sources.
• Assist clients in connecting to resources needed to reach personal goals during treatment and in advance of discharge.
• Arrange access to public assistance, social security, food stamps and medical assistance.
• Arrange medical appointments and psychiatric evaluations as appropriate.
• Refer clients to vocational and educational programming.
• Assist clients in developing a thorough discharge plan addressing significant needs.
• Maintain contact with clients after discharge to ensure coordination with other services.
• Ensure compliance with federal/state/funding guidelines.
• Maintain valid driver's license.
• Other duties as assigned by Supervisor.
