Qualifications

Responsibilities

• Orient participants to the program, goals, and available services.
• Develop treatment plans and coordinate care with a multidisciplinary team.
• Document progress and maintain confidentiality per policy and law.
• Advocate for participants and connect them with ongoing resources after completion.
• Collaborate with families, providers, and community resources to ensure continuity of care.
• Monitor progress, address crises, and participate in program revisions.
