Job summary

Lake-Geauga Recovery Centers Inc. is seeking a dedicated Case Manager to support clients in long-term recovery from substance use and co-occurring mental health disorders.

You will coordinate referrals, assist with treatment planning, and maintain detailed documentation while collaborating with the treatment team. The role requires flexible hours, including evenings and weekends, reliable transportation, and a strong commitment to ethical care and client confidentiality in a residential setting

Qualifications

• High school diploma required; undergraduate degree in human services preferred.
• Certified Drug and Alcohol Counselor (CDCA) licensure preferred.
• Strong knowledge of substance abuse and co-occurring mental health disorders and available community resources.
• Experience in a residential treatment setting preferred.
• Excellent communication, interpersonal, and organizational skills.
• Availability to work evenings, weekends, and off-site as scheduled.
• Reliable transportation required.
• Valid Ohio driver’s license and a clean driving record.
• Minimum of 1 year in a personal recovery program, if applicable.

Responsibilities

• Assist clients in accessing health, housing, employment, and other services based on individualized treatment plans.
• Engage clients in supportive discussions and activities that promote recovery.
• Schedule and provide transportation for medical and mental health appointments as needed.
• Facilitates screening and intake and assist with assessments, treatment planning, and counseling as assigned by supervisor.
• Work with the treatment team to ensure alignment of transition plans with individualized treatment goals.
• Plan and coordinate referrals to support services, monitor progress, and advocate for clients.
• Liaise with referral sources and maintain communication to support client needs.
• Maintain operations and address emergencies independently.
• Uphold professional ethics and client confidentiality, including knowledge of client rights and grievance procedures.
• Complete client charting and required documentation promptly and accurately, including tracking outcomes for quality improvement.
• Attend agency, staff meetings and scheduled in-services.
• Maintain necessary certification and/or licensure as applicable.
