Job summary

Crossroads Inc, an Arizona licensed substance abuse treatment provider, seeks a Case Manager to support the Clinical Coordinator in client services, treatment planning, case management, and discharge planning. The role involves crisis management, coordinating care, and documentation in compliance with regulations.

Ideal candidates will have a High School diploma or GED and 1–2 years of related experience, with proficiency in EMR and Microsoft Office.

Qualifications

• Minimum High School diploma or GED.
• 1–2 years related experience and/or training or equivalent combination of education and experience.

Responsibilities

• Develops and documents individual treatment plans and monitors progress.
• Maintains a client caseload and completes documentation in a timely manner.
• Provides individual and group interventions including crisis management.
• Coordinates care with treatment teams and discharges planning.
• Participates in staff meetings and trainings.
• Assist clients with government assistance and referrals as needed.
