Crossroads in Arizona is seeking a Case Manager to support the Clinical Coordinator in client services, treatment planning, caseload management, and discharge planning. The role involves crisis management, coordination of care, and documentation to meet regulatory requirements and agency policies.

The position requires strong communication, office software, EMR proficiency, and adherence to policy while serving men, women, and Veterans with Substance Use Disorder in a fast-paced setting.

Qualifications

• Includes strong written and verbal communication skills.
• Proficient in Computer/Microsoft Office/EMR and typing.
• 1-2 years related experience and/or training or equivalent combination of education and experience.

Responsibilities

• Develops and monitors individual treatment plans and documents progress.
• Maintains a client caseload and completes required documentation timely.
• Provides individual and group interventions including crisis management.
• Coordinates care and discharge planning; reviews weekly progress with the team.
• Assists clients with applications for government assistance and referrals.
