Job summary

Zinnia Health in Rhode Island is seeking a Case Manager to coordinate group therapy, handle documentation, and support ancillary services. You will work with the Primary Counselor and Clinical Director to deliver high-quality client care.

The role requires coordinating care, drafting service plans, reviewing progress, managing discharges, and acting as an advocate for patients with insurance and finances.

Qualifications

• High School Diploma or equivalent.
• Minimum of 1+ years' experience as a group facilitator in Substance Use and Mental Health settings.
• Knowledgeable of 12 Step Recovery programs.
• Knowledge of Electronic Medical Records.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
• Draft service plans, review case progress and determine case closure.
• Communicate patient needs to care team members and follow up accordingly.
• Manage discharge plans upon completion of treatment.
• Attend clinical and administrative meetings for inter-departmental sharing.
• Collaborate with patients, families, therapists, and referral sources to ensure high-quality care.
• Facilitate multiple care aspects (case coordination, information sharing, etc.).
• Maintain the patient's comprehensive clinical record through detailed documentation.
• Advocate for patients regarding clinical status and treatment options.
• Promote quality and cost-effective interventions and outcomes.
• Record case information, complete forms, and produce reports.
• Navigate legal issues that patients may face, including court appearances.
• Adhere to professional standards as outlined by protocols, rules, and regulations.
• Arrange abeyance with social services, health, and governmental agencies.
• Perform other duties as assigned by the supervisor.
