Zinnia Health in Rhode Island is seeking an experienced Group Facilitator to lead groups addressing substance use, mental health, and related issues. You will guide clients toward insights and healthier coping, while collaborating with the clinical team to support treatment goals.

The role involves preparing curricula, documenting progress notes on the day of service, and ensuring care is patient-centered and evidence-based within a 40-hour work week.

Qualifications

• Experience as a group facilitator in Substance Abuse and Mental Health settings.
• Knowledge of regulations concerning alcoholism and drug addiction treatment.
• Familiar with 12 Step Recovery programs.
• Ability to document accurately and maintain client records.
• Strong communication and collaboration with treatment teams.

Responsabilités

• Facilitate both process and topic groups related to substance abuse and co-occurring disorders.
• Prepare group curriculum relevant to the topic.
• Communicate with treatment team and other staff as needed to support clinical goals.
• Complete group progress notes on the same day of service that provide detail on each client’s participation and highlights medical necessity.
• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient-centered.
• Attend clinical and administrative meetings for inter-departmental sharing of information.
• Work collaboratively with patients, families, therapist, and referral sources to ensure high-quality care.
• Record cases information, complete accurately all necessary forms and produce statistical reports.
