Job summary

Behavioral Healthcare Partners of Central Ohio, Inc. provides case management to clients with severe mental health and substance use issues. We implement monitoring systems, determine client needs, and ensure delivery of needed treatment and services.

The role supports ISP development, collaboration with care teams, and advocacy for housing, finances, education, and community services to help clients achieve independence and recovery. On-call duties and travel may be required.

Qualifications

• Associate’s Degree in Human Services or related field is required with 1–3 years of case management experience.
• CPR/BLS Certification required; QMHS preferred/required.
• State of Ohio Driver’s License is required or recommended.

Responsibilities

• Provide case management to clients with mental health and substance use issues.
• Develop collaborative Individual Service Plans (ISP) with treatment teams.
• Advocate for client housing, finances, education, and community services.
• Provide emotional support and monitor health, treatment appointments, and medications.
• Coordinate transportation to appointments and community resources.
• Maintain client confidentiality and complete progress notes in ISP documents.
