Job summary

Behavioral Healthcare Partners of Central Ohio, Inc. (BHP) is seeking a dedicated case manager to support clients with severe mental health and substance use disorders. You will develop ISP plans, coordinate services, and advocate for housing and supports while maintaining client confidentiality.

Responsibilities include monitoring treatment progress, linking clients to community resources, and documenting outcomes in ISP records. A strong team approach and knowledge of HIPAA are essential.

Qualifications

• Associate’s Degree in Human Services or related field and 1–3 years of case management experience for individuals with mental health or substance use issues.
• QMHS certification required; valid Ohio driver’s license and CPR/BLS certification.
• Demonstrates strong documentation, HIPAA compliance, and confidentiality.
• Ability to advocate for clients to access housing, financial help, education, and other services.

Responsibilities

• Provide case management to clients with severe mental health and substance use issues.
• Develop and update Individual Service Plans (ISP) with clients and teams.
• Advocate for housing, employment, education, and community resources.
• Monitor treatment progress and ensure delivery of needed services.
• Maintain client confidentiality and complete required documentation on time.
