Job summary

The Salvation Army in Chico, CA is seeking a Case Manager to support ARP and TLP participants under the supervision of the Program Director. The role involves outreach, information and referral services, case management, and follow-up to assist individuals through rehabilitation and recovery.

Responsibilities include needs assessments, intake processing, coordinating with agencies, maintaining confidential records in BITS, and facilitating groups and services as required by the program.

Qualifications

• AA/BA in social work, psychology or chemical dependency or currently enrolled
• Certification in Chemical Dependency Studies or equivalent preferred
• Valid California Driver License and ability to drive Salvation Army vehicle

Responsibilities

• Process applicants and manage casework for participants
• Conduct needs assessments and intake for eligible participants
• Identify outreach and referral arrangements with agencies
• Maintain confidential case notes in BITS and ensure proper documentation
• Coordinate with counselors and staff to monitor progress and assist with discharge planning
• Drive Salvation Army vehicle for appointments andTransportation as needed
