Job description

A. BASIC PURPOSE

The Case Manager works directly with the Program Director to provide supervision and support to the individuals and/or families, conduct and facilitate outreach, provide information and referral services, case management, classes and group facilitation and follow up services. The purpose of the Case Manager is to assist men and women who come to The Salvation Army Adult Rehabilitation Program (ARP) and the Transitional Living Program (TLP) for assistance with their physical, emotional and spiritual needs through a structured program of rehabilitation, work therapy, and spiritual regeneration.

B. ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Processing applicants, managing casework for program participants and supporting the counseling staff to insure program compliance.

2. Conduct comprehensive needs assessments and facilitate the intake of eligible participants with the program.

3. Identify and contact potential participants through outreach and referral arrangements with local agencies, organizations, and correspondence.

4. Open, sort and process incoming applications and screen for program recommendation or denial.

5. Prepare outgoing mail, maintaining correspondence for all inquiries, follow-up, sending applications, letters of acceptance or denial.

6. Maintain correspondence with law enforcement agencies, court officials and county offices to initiate and maintain case-management, progress reports and discharge summary.

7. Meet with counselors to assess and monitor progress of beneficiaries in completing the program expectations and accomplishment of personal objectives in recovery; guide and facilitate the beneficiaries’ participation in supportive services; provide advocacy and support.

8. Work with the participants, staff and referring agencies, as appropriate to develop program admissions and exit plans.

9. Conduct or facilitate groups, coursework materials, house meetings, and other onsite services as needed for both the ARP and TLP.

10. Input complete and accurate case notes in BITS on a timely basis.

11. Maintain confidentiality in accordance with established policies and laws in order to help maintain the integrity of the total program(s).

12. Follow all security procedures as it relates to ARP & TLP confidentiality and record retention.

13. Maintain accurate and commonly understood ARP & TLP program filing system for all incoming beneficiaries and/or participants.

14. Assist with compiling all necessary information, reports and forms as assigned.

15. Conduct UA testing and maintain accurate logs as required by law.

16. Attend scheduled in-services, workshops and staff development as required by Program Administrator.

17. Drive TSA vehicle when needed for transporting beneficiaries to appointments with outside agencies, completing appropriate vehicle compliance checks and paperwork.

18. Provide case management in county benefit eligibility and food-stamp assistance program.

19. Other as assigned

C. KNOWLEDGE, SKILLS, ABILITIES AND OTHER QUALIFICATIONS REQUIREMENTS

1. AA/BA in social work, psychology or chemical dependency) or currently enrolled

2. Certification in Chemical Dependency Studies, Certification of Alcohol and Drug Counseling with recognized State Certified Agency preffered

D. CERTIFICATES, LICENSES, REGISTRATIONS

1. Must possess a valid California Class C Driver License, and ability to drive a Standard Salvation Army vehicle.

2. Must be 21 years or older.

3. Must be able to successfully pass a criminal background check to include a DOJ Livescan Fingerprint check.

4. Complete The Salvation Army vehicle course training.

E. PHYSICAL REQUIREMENTS:

1. Ability to sit, walk, stand, bend, squat, climb, kneel, and twist on an intermittent or sometimes continuous basis

2. Ability to grasp, push, and/or pull objects

3. Ability to reach overhead

4. Ability to operate telephone

5. Ability to lift up to 25 lbs. for administrative positions

6. Ability to operate a computer

7. Ability to process written, visual, and/or verbal information

8. Ability to operate basic office equipment and tools PC, Fax Machine, Telephone, Calculator, Copier, Printer.
