Job summary

Boston Medical Center seeks a Care Coordinator (Recover Case Manager) to lead the case management of treatment plans for patients, under the direction of Operations for Stabilization Services. You will work with a multidisciplinary team to ensure access to community-based resources, CSS, and aftercare.

The role emphasizes coordination with community providers, advocacy, and support services to facilitate stabilization and long-term recovery for patients in the Brockton area.

Qualifications

• Bachelor's Degree in Social Work or related field.
• Case Management Certificate preferred
• At least 1 year working in a Behavioral Health setting.
• Knowledge of the addiction treatment continuum and related community-based resources
• Experience in treatment settings for substance use disorders
• Strong knowledge of dual diagnosis treatment

Responsibilities

• Assist patients in coordinating medication assisted treatment for stabilization and ongoing recovery.
• Coordinate care with community providers and aftercare services upon discharge.
• Enroll patients into complex care management for Accountable Care Organization members.
• Provide resources for public benefits such as DTA, SSI/SSDI applications.
• Contribute to treatment plans and discharge summaries within a multidisciplinary team.
• Advocate for patients by coordinating with probation, parole, and judicial systems.
• Connect patients with information on insurance, finances, and benefits.
• Initiate family meetings for care coordination as needed.
• Provide aftercare follow-up calls up to 30 days post discharge.
• Offer brief counseling in individual or group settings to support recovery.
• Follow BSAS, DPH and related guidelines including 42 CFR.
• Maintain professional boundaries with patients.
• Other duties as assigned.
