Job summary

Casa Esperanza, Inc. in Boston, MA seeks a Case Manager to provide comprehensive case management and care coordination for patients and their children in the LyN Residential Treatment Program.

You will coordinate medical, behavioral health, social services, and community resources to promote recovery and continuity of care. You will work with a multidisciplinary team, develop service plans, and ensure timely access to services while maintaining compliance with regulations.

Qualifications

• Experience in advocacy for children and families
• Experience with homelessness, mental illness, domestic violence, HIV/AIDS, and child case management
• Experience with motivational interviewing and trauma-informed care, preferred

Responsibilities

• Provide ongoing case management and care coordination for patients and their children.
• Develop and monitor patient-centered service plans with the treatment team.
• Coordinate medical, behavioral health, dental, psychiatric, and specialty appointments and transportation.
• Collaborate with internal and external providers for timely access to services.
• Manage DCF-related responsibilities including reunification planning and collateral contacts.
• Coordinate referrals to housing, WIC, employment, childcare, domestic violence resources, and aftercare support.
• Assist patients in meeting legal obligations (court, probation, parole).
• Facilitate psychoeducational and recovery-focused groups.
• Participate in multidisciplinary meetings and treatment planning.
• Maintain documentation in EHR including progress notes and discharge planning.
• Provide crisis intervention and patient advocacy; address barriers to treatment.
• Maintain confidentiality and comply with regulations.
• Perform other duties as assigned.
