High Point Inpatient Campus in Hyannis, MA seeks an Aftercare Coordinator to provide direct resident care, conduct assessments, develop discharge and aftercare plans, and maintain comprehensive medical records. The role supports residents through group sessions, counseling, and coordination with aftercare services.

Responsibilities include referrals, documentation, and collaboration with the treatment team to ensure continuity of care.

Qualificações

• Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion.
• Knowledge of basic computer skills and software including the ability to learn HPAO Electronic Health Record (EHR).
• Demonstrated flexibility and ability to perform multiple tasks.

Responsabilidades

• Maintain assigned caseload; provide direct care to residents including assessments and discharge planning.
• Make referrals and link clients with aftercare services.
• Coordinate aftercare services, appointments and referrals; participate in MDT meetings.
