Qualifications

• Preferred associate's degree in human services or related field.
• Experience in aftercare coordinating and community support services preferred.
• Must be able to read, write, speak, and understand English.
• Ability to make independent decisions and work with diverse populations.

Responsabilités

• Assess resident needs to determine aftercare goals.
• Develop aftercare plans to improve residents' well-being.
• Help residents apply for government benefits.
• Develop and implement measurable aftercare plans.
