High Point Inpatient Campus – Hyannis, MA is seeking qualified staff to provide direct care and counseling to residents with substance use and mental health needs. Roles include caseload management, assessments, discharge planning, and aftercare coordination, with duties spanning documentation, group facilitation, and crisis response.

The position requires a High School diploma or GED (preferred) and CPR certification.

Qualifikationen

• High School diploma or GED preferred.
• Experience in substance use treatment and/or mental health.
• CPR certification preferred.
• Knowledge of HIPAA and confidentiality guidelines.
• Ability to work with residents and families.

Aufgaben

• Maintain assigned caseload and provide direct care to residents.
• Conduct assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, and group note completion.
• Make referrals when appropriate and coordinate aftercare services post-discharge.
• Maintain medical records with professional, complete, timely entries.
• Engage with community partners to support sobriety and housing goals.
• Attend unit meetings, case conferences, and mandatory trainings.
