Job summary

The Forward Trust is seeking a motivated RECONNECT Practitioner to support people leaving prison with health, wellbeing and community integration services. You will conduct comprehensive assessments, develop person-centred SMART care plans and coordinate care with internal teams and external partners to ensure a safe transition back into the community.

The role includes up to six months post-release support, remote working with travel to prisons, offices and partner locations as required, and is

Qualifications

• Experience in mental health, wellbeing or related services.
• Experience completing comprehensive assessments.
• Experience designing SMART care plans.
• Experience delivering structured interventions.
• Experience facilitating groups is desirable.
• Skilled with motivational interviewing in 1:1 and group settings.

Responsibilities

• Process referrals and provide timely updates to referrers.
• Conduct comprehensive needs assessments.
• Develop and implement person-centred SMART care plans.
• Maintain contact with service users pre-release and up to six months post-release.
• Ensure connections to health, wellbeing and community services.
• Build relationships with external agencies and stakeholders.
• Maintain risk assessment and safeguarding processes.
• Maintain records in line with policies and data protection.
• Contribute to team targets while maintaining service delivery.
