Job summary

New Freedom Ops LLC is seeking a Re-Entry Specialist to support adults navigating recovery and re-entry after incarceration. You will work with clinical and peer teams to assess needs, develop transition plans, and coordinate care to help members build independence and stability.

Primarily onsite with travel to various institutions, the role emphasizes case management, coordination with treatment teams, and advocacy for services such as housing, employment, and insurance.

Qualifications

• Excellent communication, organizational, time management and critical thinking and problem-solving skills required
• Able to sit for prolonged periods and work in dynamic environments
• Peer and Recovery Support requirements and certifications as listed in job post

Responsibilities

• Delivers high-quality case management to support members' recovery and re-entry goals
• Communicates and coordinates care with treatment team members
• Partners with members for goal attainment including employment, housing and community involvement
• Advocates for members with respect to insurance, financial assistance and service accessibility
• Supports transition and discharge planning upon program completion
