Centurion Health is seeking a full-time Transitional Healthcare Facilitator for Rockville Correctional Facility in Rockville, Indiana. The role develops and implements transitional healthcare plans for inmates scheduled for release and coordinates with the Indiana Department of Correction and local agencies.

Requirements include a Bachelor's degree in psychology, social work, or related field, plus knowledge of community programs; two years in re-entry services preferred.

Qualifications

• Bachelor’s degree in psychology, social work, counseling or related field is required.
• Experience with/knowledge of community and state programs and services is preferred.
• Two (2) years of experience in re-entry services or release planning preferred.

Responsibilities

• Develop and implement transitional healthcare plans for incarcerated individuals scheduled for release.
• Coordinate with Indiana Department of Correction and other agencies to ensure pre-discharge planning.
• Ensure re-entering individuals are connected with necessary services and resources.
