Job summary

NeighborHealth is seeking a PACE Registered Dietitian (RD) to join the Interdisciplinary Team. The RD conducts clinical nutrition assessments, develops care plans, coordinates interventions, and oversees nutrition competencies among Dietary Assistants and Health Aides within the PACE Center.

The role reports to the Nutrition Manager and supports quality clinical care. The position emphasizes evidence-based medical nutrition therapy, culturally competent counseling, and collaboration with

Qualifications

• BS or MS in Dietetics or related field.
• At least one year as a Registered Dietitian.
• Experience with elders.
• RD/RL or RD eligible from an accredited institution.
• Licensure within 3 months of hire.
• Maintain state and national registrations.
• ServSafe Certification (Massachusetts allergen training) preferred

Responsibilities

• Completes nutrition assessments and implements care plans at enrollment and annually per policies.
• Documents progress notes and updates care plans and status.
• Provides chronic care management based on participant goals and evidence-based guidelines.
• Assesses high-risk nutrition issues and manages weight loss and related conditions.
• Collaborates with the IDT to develop and implement care plans.
• Delivers culturally, linguistically, and age-appropriate nutrition counseling.
