Job summary

You will assess strengths and functioning to craft recovery-oriented service plans and complete applications for needed benefits and entitlements. Under the Director of the TCMU, you’ll link clients to available resources, maintain contacts, and advocate for access to services, with on-call duties and a flexible schedule, supporting the goal of reducing

Qualifications

• Bachelor's degree in Behavioral Health or two years of relevant adult mental health experience.
• Active participation on a treatment team, preferably in community-based settings.
• Alternate: high school diploma with five years of mental health experience and certification.

Responsibilities

• Provide rapid outreach and engagement to referred individuals.
• Assess strengths and functioning to develop client-centered plans.
• Complete applications and linkages to needed services and entitlements.
• Investigate incidents of abuse, neglect, or rights violations within DBHIDS.
• Maintain current resource information and access processes.
• Advocate for access to necessary services and supports.
• Maintain availability via pager, cellphone, and on-call schedule.
• Demonstrate flexibility to take on additional related duties.
