Job summary

Elwyn in Pennsylvania seeks a Mobile Crisis Specialist to respond to mental health crises, assess needs, and coordinate care with community supports to reduce unnecessary hospitalizations.

You will work with a multidisciplinary team, document thoroughly, and advocate for individuals served while maintaining safety and compliance protocols. This role requires driving as part of field duties and on-call readiness.

Qualifications

• Bachelor's degree in a human service field; Master's degree preferred.
• One year of mental health direct care experience preferred.
• CAC/CAC eligible status or related recovery experience preferred.
• Demonstrated ability to work effectively as part of a team.
• Strong attention to detail and time management.
• Excellent judgment with ability to solve problems independently.
• Proficiency with Microsoft Office and EHR systems.
• Valid driver's license with acceptable driving record.

Responsibilities

• Assess individuals in crisis and secure appropriate services, including referrals and involuntary processes as needed.
• Coordinate with community supports and discharge planning to ensure continuity of care.
• Document intakes, progress notes, and treatment plans in a timely, regulatory-compliant manner.
• Advocate for individuals served with external agencies and families; communicate needs clearly.
• Maintain communication with physicians, therapists, and other providers involved in care.
• Provide crisis intervention and CPR following agency protocols.
• Attend team meetings and contribute to interdisciplinary care plans.
• Conduct safety checks, drills, and required training to stay compliant.
