PMHCC, Inc. seeks a Rapid Response Team Specialist to join the Rapid Response Team, a project within the Targeted Case Management Unit in Philadelphia.

The role focuses on quick assessment, linkage to services, and 10–15 day timeliness for service plans with outreach to Homeless Cafes and related engagement efforts. The ideal candidate holds a bachelor’s degree in Behavioral Health with at least two years of relevant experience, or a high school diploma with five years of experience and

Competenze

• A bachelor’s degree in Behavioral Health with at least two years’ relevant experience.
• Active participation as a member of a treatment team, preferably community-based.
• Or a high school diploma with a minimum of 5 years’ experience and certification for investigations.

Mansioni

• Provide rapid outreach and engagement to individuals referred to the RRT.
• Assess present strengths and functioning to develop client-centered recovery-oriented service plans.
• Complete applications for and perform linkages to needed services, benefits, and entitlements.
• Investigate incidents involving abuse, neglect, exploitation, or rights violations of individuals served.
• Maintain current information on available resources and procedures for accessing them.
• Advocate for access to services and supports needed for success.
• Maintain availability through pager, cellphone, and e-mail; participate in on-call schedule.
• Perform other related duties as assigned.
