Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide rapid response hospice field visits throughout Palm Beach County. The schedule is Fri/Sat/Sun 7am–7:30pm, offering meaningful, patient-centered care during critical moments.

Qualificações

Florida-licensed Registered Nurse required.
Current Basic Life Support certification required.
Minimum 1 year in public health, home nursing, oncology, medical/surgical, geriatric, or hospice nursing preferred.
Understanding of hospice nursing: case management, crisis intervention, communication, teaching, thanatology, and needs assessment.

Responsabilidades

Assess, triage, and manage urgent and emergent health conditions in home/community settings.
Operate with autonomy while collaborating with physicians, APs, and interdisciplinary teams to deliver timely, high-quality care and prevent ED visits or hospitalizations.
Advocate for patients and families to address needs.
Coordinate comprehensive care under direction of the primary physician and interdisciplinary team.
Monitor patient conditions, provide interventions, and deliver education to enhance comfort and quality of life.
