Job summary

Agate Housing and Services seeks a Rapid ReHousing Case Manager to help families exiting homelessness move into permanent housing quickly. This role includes intensive housing search, in-home stabilization, and ongoing support for up to nine months after housing is secured.

You will coordinate with Hennepin County Coordinated Entry, work with landlords, and partner with agency staff to overcome barriers to housing stability and income growth. A valid driver’s license is required.

Qualifications

• Must have valid Driver's License and pass DHS background checks.
• Must have vehicle liability coverage of 100/300/100.
• One of: BA/BS/AS in health or human services; 1 year of relevant experience; or Certified Peer Specialist per Minnesota statutes.
• Eligible unlicensed personnel requirements per Minnesota statutes, sections 144A.43 to 144A.483.

Responsibilities

• Assist families experiencing homelessness in transitioning to stable housing in Hennepin County.
• Search for housing and negotiate with landlords on behalf of participants.
• Provide structured case management with a focus on housing stability and income growth.
• Maintain regular contact with landlords and cultivate relationships to secure housing.
• Complete accurate case notes and data documentation for assigned clients.
