Job summary

Agate Housing and Services is seeking a Rapid ReHousing Case Manager in Minneapolis to help families move from homelessness to permanent housing. You will provide intensive housing search assistance, perform in-home case management, and maintain contact with landlords to secure stable tenancies.

The role includes coordinating with Hennepin County Coordinated Entry, visiting homes, and ensuring documentation and reporting are completed timely.

Qualifications

• Must have valid Driver's License and pass DHS NetStudy 2.0 background checks.
• Vehicle liability coverage of 100/300/100 is required.
• Education or experience in health/human services or related field.

Responsibilities

• Help families transition from homelessness to stable housing in Hennepin County.
• Search for housing and negotiate with landlords on participants' behalf.
• Provide structured case management with home visits after housing is secured.
• Address barriers to housing stability and help increase income.
• Connect families to ongoing community resources after services end.
• Build and maintain relationships with landlords and housing providers.
• Mediate landlord-tenant issues and educate on rights and responsibilities.
• Coordinate with agency staff and shelter partners and document expenditures.
