3rd Street Youth Center & Clinic is seeking a Case Manager for Rapid Rehousing of Parenting TAY in San Francisco. You will provide comprehensive case management, secure housing documentation, and connect clients with resources to support long-term stability.

Responsibilities include home visits, plan development, and data entry in HMIS, while collaborating with Housing Navigators to help clients achieve housing retention and well-being.

Qualifications

• Bachelor's degree or equivalent work experience.
• 2-3 years of work with high-risk and homeless youth.
• Experience with HMIS/ONE data systems preferred.

Responsabilités

• Provide case management and oversight for TAY from Coordinated Entry.
• Create housing retention plans and linkages to resources.
• Coordinate referrals for behavioral health, employment, education, benefits.
• Document case notes and data in HMIS/databases.
• Support Housing Program Manager and Navigators in Rapid Rehousing.
• Develop independent living and income-stability plans with youth.
• Attend housing meetings and maintain client records.
