Job summary

Delta-T Group North Jersey, Inc. seeks a Rapid Rehousing Case Manager to serve clients in the New Haven, CT area. This full-time role focuses on assessing needs, assisting with housing vouchers, and securing apartments for clients experiencing homelessness.

You will document eligibility, coordinate referrals for mental health and addiction services, support resume writing and job search, and track client progress even after housing is secured. Hours: Mon-Fri 8:30–4:30.

Qualifications

• Bachelor's degree strongly preferred, housing case management experience also considered.
• Case management experience in supportive/transitional housing is required.

Responsibilities

• Identify the needs of clients and establish social services support to assist them as they receive their new housing.
• Identify eligibility for housing, assist with housing voucher applications.
• Assist with finding and securing apartments for clients.
• Screen documents for homeless clients, including sensitive information.
• Assist with documentation for Medicaid, housing subsidies, and related entitlements.
• Initiate referrals for mental health and addiction treatment services as needed.
• Encourage social supports and community connections.
• Assist with resume writing, job searching, and interviewing skills.
• Continue Case Management after housing is secured to support ongoing success.
