Job summary

Trinity Services, Inc. in Joliet, IL is hiring an Access and Care Navigator to serve as the first contact for behavioral health services.

You will respond to service requests, conduct screenings, and coordinate admissions across Network 9 programs to ensure timely access to care. The role requires at least a bachelor's in a behavioral health field and 2+ years in related settings, with strong empathy, organization, and collaboration skills.

Qualifications

• Bachelor's degree in Counseling, Psychology, Social Work, Human Services, or a related behavioral health field is required.
• Minimum of two years of experience in behavioral health, care coordination, intake, case management, crisis intervention, or related setting preferred.
• Experience with behavioral health screenings, referrals, or navigation within behavioral health systems preferred.
• Experience with EHRs, scheduling systems, Medicaid, and private insurance preferred.

Responsibilities

• Respond to requests for behavioral health services by conducting initial screenings and determining service needs.
• Meet with individuals quickly to complete consents, screenings, assessments, and intake documentation.
• Coordinate referrals and appointments across Network 9 programs to ensure timely access.
• Manage centralized wait lists by monitoring capacity and facilitating placements.
• Collaborate with clinicians, providers, schools, hospitals, and community partners to coordinate admissions.
• Educate individuals and families about services, eligibility, insurance, and treatment options.
• Maintain accurate documentation per policies and Medicaid/regs.
• Participate in quality improvement to reduce wait times and improve access to care.
